Who can get the report
For 60 days after the crash, Florida releases crash reports only to the parties involved, their insurers, their attorneys and the others listed in Florida Statute 316.066. If you ask by mail or in person during that period, you must sign a sworn statement, FLHSMV Form 94010.
Where to request it
The Florida Department of Highway Safety and Motor Vehicles (FLHSMV) sells crash reports through the Florida Crash Portal. The Jacksonville Sheriff's Office, like many local agencies, points requesters to the FLHSMV portal.
What you need
Have the report number if you have it, along with the date of the crash and the name of a person involved. During the 60-day period, mail and in-person requests also need the signed Form 94010.
Fees and timing
Each report costs $10, plus $2 per transaction. You can buy up to 10 reports in one transaction. A report can take up to 10 days to appear on the portal. After you pay, you can open the report right away, and the link stays open for 48 hours.
If the report is not filed yet
If your report is not on the portal 10 days after the crash, ask the agency that responded whether the officer has filed it.
