How to get your Minnesota crash report

Who can get the report

Minnesota releases crash reports only to the parties: a driver, passenger, vehicle owner or pedestrian named on the report, next of kin in a fatal crash, or an attorney or insurer with a signed release.

Where to request it

Send form PS2503, Minnesota Crash Record Request, to the Department of Public Safety Driver and Vehicle Services (DVS). You can submit it in person at DVS in St. Paul or at any full-service deputy registrar, or by mail. There is no online option.

Many reports are also kept by the agency that investigated. The Minneapolis Police Department releases its accident reports free to authorized people through the city's Open City Portal; its Records Information Unit is at 612-673-2961.
